Privacy Policy

Pursuant to federal regulations promulgated under the authority of the Health Insurance Portability and Accountability Act of 1996, A Second Chance, Inc. herein states that it does not use confidential client health information, which at all times includes any and all forms of health care treatment, or billing information, including but not limited to client identity, except as permitted or required by law. This Agency does not divulge, disclose, or communicate in any manner, any protected health information to any third party without the prior written consent of the individual. In furtherance of this policy, We will take all necessary administrative and technical measures to protect such information an treat it as strictly confidential. Any employee who knowingly and/or willfully violates this policy is deemed to act outside the scope of his/her duties.

A Second Chance, Inc. maintains and encourages the use of appropriate physical, administrative, and technical safeguards to prevent the use or disclosure of any protected health information, including the identities of its clients, other than as required by law or contractual obligation with the Office of Children, Youth and Families, Department of Human Services, Allegheny County.

It is the express intention of A Second Chance, Inc. to comply with the standards for privacy of protected health information promulgated by the United States Department of Health and Human Services. If an individual, or a person acting on behalf of an individual, is dissatisfied with the measure or quality of privacy and confidentiality protection afforded to them, there are steps to file a complaint. There shall be no retaliation against an individual, or a person acting on behalf of the individual, for expressing dissatisfaction or filing a formal complaint in relation to the privacy policy and practices of A Second Chance, Inc.

If an individual is dissatisfied with the privacy policy or practices of A Second Chance, Inc., he or she, or a person acting on their behalf, such as a legal guardian or representative, may file a complaint, in writing or otherwise, expressing dissatisfaction directed to the Employee Relations Department-HIPAA Compliance, A Second Chance, Inc.
